Therapeutic alternative appendix: list the pharmacologic and non-pharmacologic options for the treatment of type 2 diabetes. Non-pharmacologic options: metformin (glucophage, glucagon-like peptide 1 receptor agonist (glp-1, medical nutrition therapy (mnt) Moderate 150min/week (50-70% max hr), intense exercise 3 days week with no more than 2 days b/t exercise. Resistance training 2x/week: weight loss, sodium glucose co-transporter 2-inhibotors (sglt-2) Initial weight loss of 5: smoking cessation, patient educations. Basal: compare the pharmacologic options with respect to efficacy, onset, safety & contraindications, drug interactions, convenience and cost (in sentence form, do not hand in a drug table. ) Sglt2-i reduce a1c by 0. 7-1% and reduces progression of neuropathy. Sulfonylureas rapidly reduce bg & reduce a1c by 0. 8%. Alpha-glucosidase inhibitors reduce a1c by 0. 6%, modestly reduce bg, not recommended as initial treatment w/a1c >8. 5%. Tzd have longer duration of glycemic control with monotherapy than metformin or glyburide. Metformin has side effects like gi disturbances (nausea, diarrhea, abdominal discomfort), decrease b12 and lactic acidosis (rare).
