The first three of the above are used in tracking growth during childhood. The first signs that a child has a serious illness is the fact that they are not growing properly. Growth for the individual child is recorded on a standard chart either who (united nations funded) or cdc (united states) form by the nurse or health practitioner. 5th, 10th, 25th, 50th, 75th, 90th and 95th percentiles are commonly shown. 50th percentile does not represent the average height or weight, but rather the measurement at which 50% of the population are recorded.
