Most clients with mental health issues are treated in the community (PCP, Psychologist, Therapist,
Psychiatrist). Admission to hospital usually occurs in a time of danger or crisis.
Reasons for admission into a hospital for mental health issues:
1. Client feeling unsafe
2. Having lost control to feel safe
3. Lost will to live
4. Increased self-harm behavior/ suicidal attempt, or homicidal tendencies
5. Failed many trials of medications
6. Experiencing symptoms of psychosis
7. Crisis situation
8. Inability to care for self
How does one become admitted to a mental hospital?
1. Civil committal (voluntary or involuntary, or informal {SDM admits person})
2. Criminal committal (court order under mental health act/ criminal code)
Forms
Form 1= admission for 72h
Form 2= police pickup and admit patient; only for ER evaluation
Form 3= 2 week admission

Form 4= good for 1mth (can be written during Form 3 admission), a second form 4 would cause another
2mth admission, a third form 4 would cause another 3mth admission
Form 5= cancellation of form 3
Form 9= an order to return patient to hospital (good for 1mth, but can be unlimited depending on unit)
Form 30= notifies a form 3 or 4 is issued: notice to patient (patient gets the original copy, photocopy in the
chart)
Form 42= notifies a form 1 is issued
Form 47= Order for Examination Sections 33.3(1) and 33.4 (3) of the Act
Form 3 & 4 can be contested by the review board
CTO= community treatment order (under mental health act)
1. Requires dr.’s order and persons consent
2. Provides a person with a comprehensive community Tx less restrictive than a psychiatric institution
3. Valid for 6 mths, but is renewable
4. If person doesn’t comply, physician can order a Form 47, which might terminate a CTO
Brian’s Law, Bill 68 (2000):
- The legislative changes remove barriers to families, police, and social workers by ensuring that people
poring a risk to themselves or others get the care & Tx they need.

ASEPTIC= mnemonic for MSE
Appearance/Behavior; Speech; Emotion; Perception; Thought Process; Cognition.
Mood= subjective experience of emotional state
Affect= objective (observational) view of emotional state
Using subjective and objective points of view for an assessment.
Major Depressive Disorder (MDD)
Characterized by 1+ major depressive episodes (at least 2 weeks of depressed mood and/or loss of
interest in usual activities, with 4+ symptoms of depression)
Happens twice as often in women, as in men
