Review of systems:
1- General: Fever and/or chills, weight loss or gain, appetite, fatigue
(weakness), lethargy (decrease of consciousness), sleeping.
- Fever: onset, duration, frequency, degree, time (day or night),
associated symptoms (sweating, chills), rash, animal contact or
not, alleviating and exacerbating factors.
2- Respiratory: chest pain, shortness of breath (dyspnea), cough,
wheeze, sputum, hemoptysis (blood in sputum), pain when
breathing.
- Cough: onset (sudden or gradual), duration (acute{<2months} or
chronic{>2months}), time (day, night, all over the day or during
exercise), intermittent or continuous, dry or wet(productive{
color, amount, consistency{watery, loose, firm}, blood or not),
associated symptoms (fever, headache, sore throat, runny nose,
itchy eyes, wheeze, night sweat , abdominal pain, weigh loss).
*Causes of cough (respiratory infection, asthma, lung diseases
{like cystic fibrosis}, gastroesophageal reflux, TB, inhaled foreign
body, smoking, habit cough).
- Shortness of breath: onset (sudden or gradual), duration, day or
night, degree, intermittent or continuous, alleviating and
exacerbating factors, associated symptoms (fever, sweating,
abdominal pain, cyanosis).
*Causes of SOB: (asthma, respiratory infection, CHD, obesity,
enlarged tonsils).
3- Cardiovascular system: chest pain, palpitation, tightness,
shortness of breath at night (paroxysmal nocturnal dyspnea) or
when lying down (orthopnea) or at exertion, leg ankle swelling,
pain when walking (claudication), cyanosis.
