Vibrio cholera has a number of virulence determinants. They genes ae off when the organism is in the water (fresh or salt). But the acidity of the stomach turns them on. These include adhesins and the very important cholera toxin. The a subunit which causes the cell to change its behavior. The b parts (five copies of it) bind to the glycolipid receptor on the cyutoplasmic membrane. A subunit is cut into a1 and a2 pieces. The a1 enters the cytosol of the cell and activates the enzyme called adenylate cyclase. So atp is made into camp lots of it. The camp stimulates the outflow of sodium, chlorine, potassium and bicarbonate from the cell. Result: severe fluid and electrolyte losses resulting in dehyration thirsxt, metabolic acidosis, hypovolemic shock. Result is: muscle cramping, lethargy, sunken eyes, irregular heartbeat, kidney failure, coma and death. Introduction of cholera into south america and haiti part of the 7th pandemic!!
