Introduced as an alternative for children and people w/ ulcers. Mechanism of action is not well understood but avoids the effects of typical. Nsaids on blood clotting as well gastric toxicity. Acetaminophen is normally metabolized by phase 2 enzymes. Problems can force modification by phase 1 p450 enzymes that produce a highly toxic intermediate napq1. Napq1 should be rapidly detoxified by phase 2 through the addition of a glutathione which promotes elimination by the kidney. Anything that causes an imbalance between production of napq1 and it"s removal leads to acute liver damage. Grapefruit contains several compounds that interfere w/ drug metabolism. Most common cause of acute liver failure (40-60%) surpassing infectious hepatitis. Acetaminophen is widely used in other drug preparations making it hard for patients to keep track of dosage. Analogs of controlled substances intended to avoid detection. Gold standard for drug detection is gas chromatography-mass spectroscopy.
