1)Framingham Coronary Heart Disease Risk score(FRS),the point is given according to few points such as:
Age,HDL-C,TC,SBP,Smoker(4 points),Diabetes(3 points)
Interpretation: risk for having CVD in next 10 years reach 30% when total score more or equal to
-18(Male) -21(Female)
2)Risk of the patients for getting CVD:
Very high risk
(30% of risk)
-Established CV disease
-DM with proteinuria
-CKD of Stage 4 or above (GFR <30ml/min)
High risk
(20% of risk)
-DM without target organ damage
-CKD of Stage 3 (GFR>30 but <60ml)
-Very high individual risk factors such as LDL-
C >4.9,BP>180/110
Intermediate (moderate) risk
-FRS of 10 -20 points
Low risk
-FRS <10
3)INTERHEART study show that the 5 modifiable risk factors are:
-Abnormal lipids
-HTN
-Current smoking contribute to about 80% of MI,especially Smoking and abnormal
-Diabetes lipids are accounted for 2/3 of MI
-Abdominal obesity
4)Management:
a)Complete stop the smoking
b)for HTN,BP keep < 135/75mmHg
c)LDL ≤.,if has CVD the ust <.8
HDL: 1.0(Male) ,1.2 (Female)
TG <1.7
d)Obesity: 5-10% of weight loss in 6 months,maintain for 1-2 years
e)Blood sugar profile:
Fasting:4.4-7.0 ,Post-Prandial:4.4-8.5 ,Hbac≤.
f)If DM more than 10 years,Starts statin no matter what is the lipid level,no aspirin
5)Severe Hypoglycemia is those who need 3rd party help, these group of people having the mortality of:
9-20% in 1st year
49-80% in next 4 -7 years
