BIMS 201 Lecture Notes - Lecture 12: Carotid Endarterectomy, Rheumatoid Arthritis, Local Anesthetic
Document Summary
Local anaesthetic techniques are used for both anaesthesia and postoperative analgesia. They are getting popular due to advances in local anaesthetic drugs, use of ultrasound and nerve stimulator which helps in better localization of nerves while avoiding or minimizing complications. Also provides excellent postoperative pain control and improves patient outcome by avoiding complications due to inadequate pain control. Local anaesthetic techniques can be used alone or in combination with general anaesthesia depending upon the type of surgery. Advantages of local anaesthesia: preservation of consciousness during surgery: Able to assess the neurological status head injury, carotid endarterectomy surgery, diabetes, turp. Patient positioning is safer, more comfortable and damage to pressure areas & joints can be avoided. Airway and neck manipulation can be avoided, esp. in rheumatoid arthritis, or an unstable cervical spine. Able to protect own airway esp. in patients with risk of aspiration pneumonia. Able to discuss with the patient directly if there is a change of procedure.
