History taking: patient"s history may not be informative , deliberate attempt to confuse fear of stigma, slow progression may not remember. Occupation: hazardous jobs for insensitive hands & feet. Age: high endemic areas- bimodal 10-14 & 30-40 yrs. History of present illness: patches , colour - hypopigmented / reddish / copper colored, number tt (1-3), bt (3-10/20/more), bb (numerous countable), bl (numerous uncountable), bl(numerous uncountable) Sensation anesthesia/ hyperesthesia: distribution site of onset, evolution of patches. Formation of new patches/ flare up of patches. Number of episodes duration of each episodes (treatment taken) Infection malaria, filaria, sorethroat, uti, dental caries, tb. Completed / defaulted cause / declared cured/rft. Adverse reaction- dryness of skin, discoloration of skin, secretions, headache, skin rashes. Past history: h/o similar complaint in past, h/o surgical correction of any deformity, h/o diabetes, hypertension, tb, jaundice. Family history: family completed ? (fertility, health status of family member (similar complaints?)
