Second line - reserved for cmv retinitis and acyclovir-resistant strains of. Does not require phosphorylation to be active. Last line - after patients fail cmv therapy with ganciclovir and foscarnet. Chills, fever, ha, rash, gi upset, anemia, neutropenia, renal toxicity. Moa: inhibits cmv replication by targeting cmv dna terminase complex. Lack of cross resistance with other antivirals. Cause of infectious mononucleosis and associated with certain cancers. Replication occurs in the epithelial cells or the surface of b lymphocytes of the pharynx and salivary glands.
