Miss - looking for something in cache and it is not found. Direct mapped cache known for lowest hit ratio. In this case the block offset can uniquely specify 4 entries. The tag is what is left over to uniquely identify an address. When checking to see if an address is in cache first the block offset is. When checking to see if an address is in cache first the block offset is searched, then index, and if the tags are the same a hit has occurred. If instead of 24 bit address a bigger 32 bit address was used while keeping the cache dimensions the same then the tag would be bigger.
