Document Summary

To assess causal effects, we have to contrast the outcome of people exposed to a risk factor wit what would have happened in the absence of exposure. The closest we can come to such a design is an experimental study. The external validity is determined by how closely matched the study population is to the reference (or general) population. The internal validity is determined by randomization process. It is achieved when an observed difference (or lack of difference) in the treatment and control groups can be attributed to the treatment alone. Self-selection to the treatment group is eliminated: example: studying the association between hrt and heart disease. Predictive factors (known and unknown) tend to be balanced between intervention and comparison groups: known factors have been assessed in the study, unknown factors may affect the outcome, but have not been assessed in the study. An observational study of 121,700 female registered nurses which commenced in.
