Step 1: adverse drug effects (ades) cause 44,000 deaths and millions of hospitalizations a yr. research estimate that 95% of ades go unreported. Many of the reported ades are caused by hospital staff. Step 2: the ultimate goal is to use health info tech to reduce the ade problem. Step 3: ades can come from prescribing errors or patient"s lack of understanding the proper dosage. The main cause of the problem is lack of education or info from both the physician or hospital workforce and the patient. Step 4: solutions should be connected the utilizing health info tech. Step 5: recommendations include using clinical decision support (cds) systems to prescribe medication and to encourage medications adherence. Step 6: the action plan was exemplified by north carolina long-term. It used cds to improve patient health outcomes in nursing home setting.
