No overall budget since mostly privately based rationing carried out on demand rather than supply side: price rather than personnel/technology is the limiting factor in other countries supply is limiting factors is fewer machines, hospital beds. Not universal until 2014 aca, had relied on voluntary private insurance. Prior to aca: not universal, employers did not have to offer coverage.
