Acetylator status: amt of acetyltransferase for drug metabolism. Preexisting allergies, diseases, viral infections, previous drug reactions, drug-self cross reactivity. Atopic: high levels of circulating ige is a risk factor. Colon cancer ab can cross react with rbc. Many are haptens (bond covalently with host proteins) Admin site (iv and oral are tolerogenic, im is immunogenic) Examples: drugs and metabolites/haptens: (fyi, don"t need to know specific metabolites, just that they cause rxns) Adr are not like pharmacologic reactions ie) vasodilators cause dizziness, lower bp, but not a rash! Would reappear promptly after readmin of drug. Or with admin of drugs with similar structures. Potential etiology: alteration of self ag by drug/metabolism, cross reaction of self with hetero antigens. Drugs may induce immune responses to what self ag? (fyi slide: 10) Often in caucasians for a class i protein (don"t memorize: hla b5701) Hapten driven allergy, type iv t cell mediated. Skin rash, fever, eosinophilia = high eosinophils.
