Afferent and efferent messages
o Afferent sensory nerve messages: send messages associated with pain and touch or
vibration. From P up to the afferent pathway to cerebrum where they are interpreted by
the brain.
o Through the dorsal horn. Upward to the brain where the message is perceived. Then
you will want to withdraw from the pain
o The message is sent back down through the efferent messages.
o They go through the ventral horn. They control skeletal muscle responses.
Outer most layer that is protecting the spinal fluid in the spinal cord is the dura layer
o The next layer is arachnoid layer
o The next layer is the pia layer.
o The spinal fluid is in the sub arachnoid space.
Contusion or laceration of spinal cord and epidurals
Epidurals: located in the epidural space. Insert needle with a catheter. The needle then comes
out.
o Will bath the afferent and efferent nerve fibers
o Analgesic: pain medicine
o Anesthetic: causes numbness below where the epidural is.
o Not a high risk of spinal fluid leak
A spinal anesthetic.
o Put into the arachnoid space where spinal fluid is. This is will provide both anesthetic
and analgesic except this medicine is being infused into a different area
o Difference is 1. The location and 2. Its use
o Epidurals have indwelling catheter whereas the spinal anasethic, there is just fluid
injected
Bone formation
o Osteoblast: bone formation. Lay down new bone
Reklease rank ligand
o Osteoclast: bone resorbtion. Reforming or reabsorbing bone. Releases Ca ++ in blood
stream
Rank Ligand
o Purpose is to attach to protein receptor.
o Attaches to Rank receptors on osteoclasts
o When they connect, it activates the osteoclast to come down and reabsorb bone
Osteoprotegrin
o Acting like a decoy for rank ligand
o Osteoprotegrin also relased by osteoblasts. When the osteoblast releases
osteoprotegrin it acts like a decoy
o OPG attaches to rank ligand causeing rank ligand to think it has attached to the rank
receptor
o We need these decoys because we need both times when the bone is absorbed and
