Initial boom in the beginning (like new restaurant) People who know about the exchange have joined, but then there"s people who don"t know about it (like how to join) stay unjoined (no help getting in) Healthy people who join initially might leave the exchange from cost-benefit analysis (not worth it to have insurance) People who don"t know about premiums, deductibles, etc: so they buy a cheap premium plan and realize they have to pay a lot out-of-pocket (high deductible) Don"t like it, so they leave the exchange. Healthy people don"t care about how big the network is. Sick people: more likely to have pre-existing physician relationships, and need a wider network. Ex: every diabetic drug is on tier 3 (patients pay more), so patients are discouraged to sign up for this plan.
