Some pioneer acos like his brother"s: palliative doctors were seeing patients at home, patients were happier, hospitalization decreased by half, costs came down 20-30% He learned that some non-profit organizations covered half of the cost for palliative care. Aspire: contract with providers taking risks such as medicare advantage plans, private medicare, managed medicaid, commercial health plans, acos, built a medical team to see patients at home, patients can call to be seen any day at home. Take medications that allow them to stay home. They don"t see ffs patients, only managed care patients. Cms recently started to pay for advanced care planning conversations. How does the aca impact this: providers who are willing to take on risk want to partner with them, aca shifting risk from health plans to providers impacts who"s thinking about how to manage these patients.
