Document Summary

Complete the following sections of the assessment report and submit for marking and feedback. Avery (pseudonym) was referred to utut in 2017 for the intensive behavioural. His parents, who are active participants in his treatment and care, were encouraged to seek this extra support due to the increasingly challenging behaviours that were occurring at home and at school. His teachers noted that he excels academically, but that he could benefit from a reduction in protesting behaviours and need for extra supervision for daily tasks. It is expected that through these behavioural-change programs, avery will increase his attention & focus, increase communication of affect, and gain more independence. Avery is a 9-year old boy diagnosed with autism spectrum disorder. At present, avery attends utut 5 days a week from 9am to 12:30pm for intensive behaviour intervention. He would be considered to be an intermediate (advanced on certain academic skills) learner.
