The investigation uncovered highly explosive situations involving resident abuse with apparent knowledge and approval of top administrators. Jack mcallister (director of the state health and rehabilitative services) formed a 9-member blue ribbon panel resident abuse investigation. Lots of reports (1 death) came under jack mcallister.
