Document Summary
Workers were attempting to clear the blockage of a sewer backup using a vactor truck equipped with a rodding system. The crew was using the pressure hose to loosen the blockage in the pipe but the pipe could not be seen clearly. The issue with the pressure hose was that it was not equipped with a guide fin and returned and deflected off a crewmember and struck the worker. In 2011/2012 construction had the most stop work orders at 5,563 (see appendix b). This indicates that 5,563 orders were issued to stop work due to immediate danger or hazard to the health and safety of workers. Through the same time period, 60, 340 orders were issued; making construction the second most orders issued area of work (see appendix a). These orders are typically issued when an inspector believes certain legislation or regulations are not being complied with.
