This transformation has three main parts - the first is funding reform. Funding based on evidence-informed-pricing will compensate health service providers for the number of patients treated and the complexity of their care. The end result will be better value for our money, and even more important, better care for patients as they journey through the health care system. Health links will provide an opportunity for all health care providers -- in partnership with individuals, families and the community -- to collaborate in meeting the health care needs of individuals who live with very complex conditions. Health links are a significant part of the transformation of our health system.
