Family Health Team
- Family Health Teams are a collaborative primary health care service that brings
together different health care providers to coordinate the highest possible quality of
care for the patient
- Family Health Team staff can include nurse practitioners, nurses, social workers,
pharmacists, dietitians, respiratory therapists, psychologists and occupational
therapists who will work with physicians as an interdisciplinary team to provide
patient centred care
- There is no cost to patients for any of the services; All the programs and services offered
by the Family Health Team are funded by the Ontario Ministry of Health and Long-Term
Care
OT Patient Populations in the Thames Valley FHT (Chronic Disease Population)
1) COPD
o Design exercise program to improve conditioning for ADLs
o Teach energy conservation/work simplification; pacing will be more
beneficial than a rigid exercise plan; breaking down tasks to slowly build and
maintain maximal functions of ADLs and functional mobilities
o Recommend adaptive equipment/mobility equipment
2) Diabetes
Assess sensory impairment/recommend strategies or equipment to maintain
safety
Recommend adaptive equipment/mobility equipment

3) CHD & Chronic Fatigue
o Patient education, teach pacing, work simplification, energy conservation
o Recommend adaptive equipment or light weight equipment for ADLs
4) Arthritis
Provide gentle stretching/strengthening exercises recommend strategies
/equipment for functional mobility/ADLS
