Hydration is started 12 hours before conditioning and
continues for 24 hours after last dose of Bu

5mg/kg/d PO divided on 3 doses day -7 to day -2(
day -------- to day -----------)
. Premedicate with zofran 16mg +2amp decadron
+150cc D5w over 1 hour, then give every 8 hours
regularly till 24 hours after busulfan
.Fludarabine ( prefer 8AM)
Fludarabine 30mg/m2+100ml D5W over 1hour daily
by pump , followed by busulfan dose
Busulfan (Myleran tab 2mg) ( prefer 9AM)
1mg/Kg/6 hours PO ( mg) (d-6till d-3) day --/--
/200- , --/--/200- , --/--/200-, --/--/200- (grind and
dissolve in a juice)
If patient vomits within 30 min of administration and
tablet fragments noticed, repeat 100% of busulfan
dose
If patient vomits within 30 min of administration and
no tablet fragments noticed, repeat 50% of busulfan
dose
If patient vomits between 30 min-60min, repeat 50%
of busulfan dose
If patient vomits after 60min, do not repeat of
busulfan dose
NO PARACETAMOL PRODUCTS DURING INTAKE
