Chapter 1 - critical thinking in health assessment: Assessment: data collection on individual health state to make judgment/diagnosis based on data from various sources: subject data during history exam: person says about themselves, objective data during physical exam: observe by inspecting, percussing, palpation, & auscultation. Database: subjective & objective data w/ patient"s record & laboratory studies. => data sources: health state, health problem, risk factor, life processes & overall wellness level. Cues: piece of info, sign or symptom, or piece of lab data. Hypothesis: a tentative explanation for a cue or a set of cues that can be used as basis for future investigation. After data collection => develop preliminary list of significant signs and symptoms & all patient health needs: nursing process. Nursing process: problem-solving approach to clinical judgements, use based on experience/lvl: assessment, diagnosis, outcome identification, planning, evaluation. Interview: review the clinical record, health history, physical exam, functional assessment, risk assessment, consultation, review of the literature.
