Patients are not receiving quality care and are paying outstanding fees for poor care. Despite having the world"s best doctors and health care technology, we fall short in providing cohesive care to our patients (blum, 2011). One possible solution to this issue is coordinated care. Coordinated care means having the patient collaborate with their health care services, besides their primary doctor. Coordinated care allows patients to transition smoothly between care settings and for health care professionals to easily assess and treat the patient through effective communication and collaboration (splan, 2015). Poorly coordinated care leads to hospital readmissions, too much care, and ineffective payment plans, which all increase the healthcare bill. The half of the total rising healthcare costs come from the care we provide to five percent of the population; these are the patients dealing with chronic illnesses, mental and behavioral health issues, and other complex issues (fund, 2013).
