Document Summary
Sepsis: definition: interchangeably called septicaemia, sepsis, toxic shock, blood poisoning, toxaemia, bacteraemia. Epsis should (cid:271)e defi(cid:374)ed as (cid:862)life threate(cid:374)i(cid:374)g orga(cid:374) dysfu(cid:374)(cid:272)tio(cid:374) (cid:272)aused (cid:271)y a dysregulated host respo(cid:374)se to i(cid:374)fe(cid:272)tio(cid:374)(cid:863). Important to try and remove secretions but may need to sit up high, if breathless at rest and high respiratory rate cannot be flat or be tipped down. Direct lung trauma: aspiration, acid (gastric contents) Only steam is capable of inflicting direct thermal damage. Most injury is caused by incomplete products of combustion. Changes that occur: pain, decreased movement of ribs (or paradoxical breathing) Decreased gas movement o2: generalised, localized (contused area, causes decreased pao2. Increased load +- decreased ability to cope with the load: decreased efficiency, sternal fracture, flial segment, diaphragm rupture. Long term 4/12 post injury: decreased frc, decreased pefr, chest wall deformity. Pulmonary contusion: bruised lung not confined to anatomical segments, becomes more oedematous over the first 48 hours, ct: pulmonary infiltration + consolidation.
