Domains (hallucinations, delusions) > rate on scale of (equivocal severe) (present / not present) Generic series of question the therapist works through, but can veer off in different directions. Onset questions asking about onset (how did this begin) Psychological tests: eg. mmpi, intelligence tests, behavioural assessment. The d2 test of attention: asses ability to focus and maintain concentration, do they loose focus, do they slow down as their progressing through attention test.
