Acetaminophen 10-15 mg/kg/dose q4h prn (max 75 mg/kg/day) Ibuprofen 10 mg/kg/dose q6-8h prn (max 40 mg/kg/day) For children >6 months old with unilateral aom, mild otalgia <48 hrs, temp <39 c. For children >2 years old with mild symptoms and unilateral or bilateral aom. 2 out of 3 children will recover without antibiotics. Close monitoring and follow up if symptoms worsen or fail to improve in 48-72 hrs , follow up physician visit or start abx prescription. >6 months : with moderate to severe otalgia, or otalgia >48 hrs, or temp >39 c. First line for most children >6 months old: Amoxicillin 90 mg/kg/day divided bid-tid (consider high dose if daycare attendance, <3 months since last antimicrobial exposure, <2 years old) Duration: 5 days or 10 days if <2 years or perforated eardrum.
