Benzodiazepines are the most commonly used hypnotics and anxiolytics. Does not affect functioning the next day. Commonly used : diazepam, oxazepam, lorazepam, clonazepam, alprazolam, temazepam. Bdz binds to their own receptors on/coupled to gaba a receptors . Found in cerebral cortex and spinal cord in high density. Gaba is an inhibitory neurotransmitter in the cns. Barbiturates increase duration of gaba mediated cl- channel openings. Bdzs increases amplitude of gaba-mediated ipsps and lengthen decay phase proportionately . Do not activate gaba a cl- channels directly. Act allosterically to increase affinity of the receptor to gaba. In the absence of gaba, bdzs do not have an effect on gaba a cl- channel function. Barbiturates increases amplitude of gaba-mediated ipsps and lengthen decay phase greatly (4-5x normal). In high doses, barbiturates do not require gaba to be present. Can activate channel directly and keep it open. Overdose (massive inhibition) respiratory depression coma.
