Placenta previa occurs when the placenta abnormally implants in the lower segment of the uterus near or over the cervical os instead of attaching to the fundus. Abruptio placenta is the premature separation of the placenta from the uterus, which can be a partial or complete detachment. This separation occurs after 20 weeks of gestation, which is usually in the third trimester. It has significant maternal and fetal morbidity and mortality and is a leading cause of maternal death. Maternal hypertension (chronic or gestational)( most common cause) No pain painless bleeding, fresh red blood. Low lying previa = placenta is less than 2 mm from cervix. Partial previa = some part of placenta is covering cervix. Complete previa = cervix is completely covered. Ultrasound is the best way to confirm previa. Uterine scarring (previous cesarean birth, curettage, endometritis) Maternal age greater than 35 to 40 years. Administer iv fluids, blood products, and medications as prescribed.
