Alcohol and drug use disorder (aud) arises in stages: 1) initiation of drug use and continuation. Begin to experiment with different forms; learning. Binge drinking alone puts one at high risk of abuse. Alcohol/drug controls them instead of them controlling it. Aud is predicted by several youth variables: Parents and peers model or encourage alcohol use. Lower closeness with parents, parental monitoring, and discipline. Even stronger predictor than parental alcohol use or parenting variables. Due to genes involved in alcohol metabolism, many people cannot tolerate even small amounts of alcohol. Initial unpleasant alcohol reactions can prevent use. Lifetime prevalence of aud is 3 - 5x higher among parents, siblings, and children of alcoholics compared to general population. In twin studies, concordance rates are 13 - 23% higher in identical twins that in fraternal twins. Children of aud parents adopted by non-aud parents still have higher rates of aud.
