Integrated delivery systems (ids) just mentioned above like home health and skilled nursing care. In the 1970s doctors began to accept a fixed payment for service which launched the health maintenance organization hmo. By 1990s hmos lost their momentum as consumers reacted against the restrictions they placed on choice. Meanwhile doctors formed single specialty group practices that ensured bargaining clout in price negotiations but lacked care coordination. The common funds commission on a high performance health system in 2007 called for the nation to reform waste and fragmentation that exit.
