There is some degree of coronary artery occlusion. Acute coronary syndrome occlusion must be at least 75%. Lactic acid builds up from anaerobic metabolism from hypoxia. Occurs with increasing frequency, lasts longer, with less exertion or stress. Must treat angina within 20 minutes or can evolve into nstemi or stemi. Part of the heart loses blood supply totally and that part of the heart dies. Myocardium can become hypoxic within the first 10 seconds of the occlusion. Switch over from aerobic to anaerobic metabolism so build-up of lactic acid. This causes referred pain in the left shoulder shooting down the arms bc messages sent to cardiac nerves in the upper thoracic posterior nerve roots. Occurs when there is diminished blood supply to the heart which leads myocardial cell damage. Ischemia: normally due to blockage of coronary vessels. Often result of thrombus or unstable plaque formation. Contractile function ceases in necrotic areas of the heart.
