Maria sanchez is a 43 year-old hispanic woman with diagnosis of gastroenteritis. c/o 3 day history of diarrhea and vomiting. She has been unable to eat or drink anything for the last 2 days. Diarrhea, vomiing and unable to eat or drink (could be bleeding, diaphoresis)- anything that causes increased loss or decrease intake. 3 day hx of diarrhea and vomiting and complaints of being unable to eat and drink. Restlessness, drowsiness, lethargy, confusion, thirst, dry mucous membranes, decreased skin turgor, decreased cap reill, postural hypotension, increased pulse (weak and thread), decreased cvp. I & o, measure urine speciic gravity, daily weights, iv nutrients, treat the cause something for nausea and diarrhea to stop the loss and give luid, coninue monitoring so we don"t over correct: write a nursing diagnosis. Fluid volume deficit r/t gastroenteritis aeb diarrhea, vomiting, decreased input and increase output.
