Thyroiditis is a benign inflammatory or autoimmune disorder of the thyroid gland. The subtypes of thyroiditis include hashimoto"s thyroiditis, subacute lymphocytic thyroiditis, subacute granulomatous thyroiditis, acute suppurative or infectious thyroiditis, invasive fibrous thyroiditis, drug- induced thyroiditis, radiation-induced thyroiditis, and thyroiditis associated with other disorders. Hashimoto"s thyroiditis is the most common subtype of thyroiditis. It is autoimmune-mediated and involves both cell and antibody-mediated mechanisms. It typically presents as painless diffuse goiter in young and middle-aged women and is the most common cause of goiter and hypothyroidism. The clinical presentation of hashimoto"s thyroiditis is typically an enlarged thyroid gland. Patients may be in the euthyroid stage, hypothyroid, or hyperthyroid. Diagnosis is made through thyroid function tests and examination for elevated thyroid antibody levels. Treatment involves monitoring in patients without thyroid hormone replacement and starting thyroid hormone replacement therapy in patients presenting in the hypothyroid state. The goal is to achieve a normal tsh level.
