Nitroglycerin sublingual (sl) 0. 4mg, q5 minutes up to 3x. Aspirin: bottle is sensitive to light and temperature extremes, do. Mechanism of action: not put in fridge or by heater: throw away after 6 months, should tingle with underneath tongue. Transdermal nitroglycerin (patch: keep on for 12 hours, take off for 12 hours. Iv nitroglycerin: for when sl is not effective, emergent situation. Ointment should be place on location free of hair. Peripheral vasodilation and pooling, decreases pre/after load. Headache from dilation of cerebral blood vessels. If chest pain is still unrelieved, consider morphine! Acts as a vasodilator to reduce preload and myocardial o2 consumption. Prevents conversion of fibrinogen to fibrin and prothrombin to thrombin. Interferes with hepatic synthesis of vitamin k-dependent clotting factors. Maintain same level of vitamin k intake. Reduce heart rate, contractility (how fast heart beats), and bp. Can cause wheezing and mask signs of low blood sugar.
