Instability is a complete loss of contact between articulating surfaces of a diathrodial joint. In an acute dislocation, assess rc function by getting individual to elevate arm, if a tear is present, elevation beyond 90 degrees is difficult. Fall with arm in full elevation with player on top/dive into phase (e. g. afl, baseball) Fall onto posterior shoulder with arm across body (e. g. when carrying a ball, trauma, rugby) Type and severity of patho-anatomical lesion influenced by patient"s age, moi and severity of trauma: Younger = labral tears/ capsular damage (as they are weaker structures) Older = rotator cuff tears (as with age, the rc become weaker) High energy trauma causes greater soft tissue and bony trauma. Bankart lesion: detachment of anterior labrum and anterior capsule: displacing anterior labrum reduces glenoid depth by 50% which therefore reduces the concavity/compression mechanism lost, plastic deformation of capsule is also fundamental (may need plication) Incidence of bankart lesion after acute dislocation (87-100%)
