Taking patient vital signs is a crucial part of integrate nursing care. The purposes of
taking vital signs can be listed as follows:
a. To know the fundamental situation of patient
b. To recognize early signs of deteriorating patient
c. To make a better nursing plans
d. To ensure patient safety
describe normal ranges for vital signs
Heart Rate: 60-100 bpm
Respiration Rate: 12-20 bpm
Blood Pressure: systolic 100-130 mm/Hg
Diastolic 60-90 mm/Hg
Temperature: 36-38 degree Celsius
SpO2: 95-100 %
MET call criteria (depends on the hospital policy)
BP
120/80
Inform doctor if systolic <100/ >180, if diastolic <30 / >100. In case
of symptomatic, take another time or do the manual BP.
HR
60-100
Inform doctor if <60 or >130. If symptomatic, do manual HR.
RR 12-20
Inform doctor if <12 or > 30 or in respiratory distress.
TEMP
35.6-37.5
Recheck if < 355.5 + inform doctor if > 37.8. (Panadol if 37.5)
SaO2
95-100
Normal for COPD patients >90. In some patients aim 88-92%.
put O2 if <90 depending if CO2 retainer.

HR: tachycardia, bradycardia
RR: tachypnoea, bradypnoea, apnoea
SPO2: hypoxia, hypoxaemia
BP: hypertension, hypotension
TEMPT: afebrile, febrile, hyperthermia
