MEDI7305 Lecture Notes - Lecture 12: Febrile Seizure, Aedes Aegypti, Yellow Fever
M4d - Zoonoses & Tropical Diseases in Rural Setting
Mosquito-borne viruses
RRV, BFV Alphavirus
Chikungunya, Sindbis, Getah, Ross River virus, Barmah forest virus
Average cases/yr in QLD & cases as % Aus case
RRV 1225 cases per year in QLD (2010-2014) -> 45% Aus cases
BFV 662 cases per year in QLD (2010-2014) -> 57% Aus cases
Notifiable disease - YES
Geography of infection
Located all around QLD (also rest of Aus)
Transmission - source, contact, vector, route
Route - mosquito bites, multiple species
Incubation - 5-10 days (ranges from 3-21 days)
Symptoms + signs
Similar presentations for RRV vs BFV
Fever, fatigue, headaches, lymphadenopathy
Painful, symmetrical polyarthritis or migratory polyarthralgia (usually ankles,
knees, fingers, elbow, feet and wrists)
Variable viral exanthem with rash ranging from few spots (hands/feet) -> whole
body involvement
oTypically sparse 1-5mm maculopapular lesions on limbs & trunk
oOccasionally dense rash +/- purpuric areas +/- small vesicles
oBuccal and palatal lesions can occur
Investigations
Diagnostic tests - RRV and BFV serology
IgM (after day 7), IgG (after day 15)
Titres of 1:1280 or higher indicate recent infection
Look for 4x rise in Ab titre on parallel tests (2-4wks apart) OR conversion from one acute
pattern to convalescent pattern (below)
