Painful, symmetrical polyarthritis or migratory polyarthralgia (usually ankles,
knees, fingers, elbow, feet and wrists)
Variable viral exanthem with rash ranging from few spots (hands/feet) -> whole
body involvement
oTypically sparse 1-5mm maculopapular lesions on limbs & trunk
oOccasionally dense rash +/- purpuric areas +/- small vesicles
oBuccal and palatal lesions can occur
Investigations
Diagnostic tests - RRV and BFV serology
IgM (after day 7), IgG (after day 15)
Titres of 1:1280 or higher indicate recent infection
Look for 4x rise in Ab titre on parallel tests (2-4wks apart) OR conversion from one acute
pattern to convalescent pattern (below)
