Strategies to improve med. safety:
1. Reduce/ eliminate possibility of error – keep only one strength or reduce access to drugs
2. Accept that errors do occur, but make them visible so it can be quickly fixed – double checking
3. Minimise the consequences of error, eg. develop system
4. Using forcing functions, eg. syringes for oral use can not fit into IV
5. Use differentiations eg. differentiate drugs that sound the same such as using Tallman letters
6. Use constraints/ approval
7. Standardise procedures (drug charts, Rx, tablets) and simplify the orders
8. Appropriate storage (separate if similar packaging), warning label on shelves
9. Use reminders, eg. check for INR if they are using warfarin
