What is drug toxicity: any noxious, unintended and undesired effect of a drug, excluding therapeutic failures, intentional and accidental poisoning and drug abuse. Serious adr: death, danger to life, admission to hospital, prolongation of hospitalisation, absence from productive activity, birth defects. Adverse drug events and adverse drug reactions and medication errors are all different. Most drug adverse reactions in elderly people occur within a therapeutic dose range and are known as collateral. Common- in australia: 10% of patients visiting a gp have an adverse drug effect, 10-30% of patients in hospital have an adverse drug reaction. 1-(cid:1007)% of deaths i(cid:374) hospital are due to ad (cid:859)s. 5-(cid:1006)0% of ad(cid:373)issio(cid:374)s to hospital are due to ad (cid:859)s. Adr is definite if the overall score is 9 or >9, probably for 5-8, possible for 1-4 and doubtful if 0. What happens when you report a suspected adr: no further action.
