Bronchial inflammation-> obstruction-> atelectasis (loss of lung volume) Air bronchogram sign: consolidation of the lung parenchyma with little/no involvement of the airways. No volume loss (because air is replaced with exudate. Involves walls, lining and support structures of alveoli (alveoli septa) The aspiration of esophageal or gastric contents into the lung. Aspiration in the right lung most often because it is more vertical. Necrotic area becomes encapsulated by a fibrous wall (contained, seen if it is advanced) but can expand. May be a complication of aspiration, bacterial pneumonia, bronchial obstruction, foreign body, blood infections or septic emboli. Spherical density- dense centre, hazy poorly defined periphery. Air fluid level communication with bronchial tree. Someone with a lot of reflux, neurological deficits that have difficulty swallowing, inability to sit up (lying on their back usually), diverticulum, fistulas, etc . Gravity- posterior parts of the upper and lower lobes are most commonly affected.
