Jwh, jm, ajs: kernicterus - bilirubin induced brain dysfunction leading to deafness and choreoathetoid cerebral palsy (however we are less worried here as it is only unconjugated bilirubin which can cross the bbb) Medical (mostly for neonates with jaundice): phototherapy e. g. (cid:862)(cid:271)ili(cid:271)lanket(cid:863) (cid:449)hi(cid:272)h (cid:272)an (cid:271)e used for 24 hours to deli(cid:448)er optimum light therapy. The light photoisomerizes the unconjugated bilirubin to a soluble form which can be excreted in the urin: exchange transfusion, there are tables which show: days postnatally and levels of unconjugated bilirubin at which the treatments should be commenced. Surgical: referral to local specialist centre, perform the kasai procedure (portoenterosotomy) earlier this is done the better the outcome. Definition: systemic vasculitis - small and medium sized vessels. Ben is a 2-year-old boy who presents to the rapid referral paediatric clinic. No contact with people who have had infections. General was fit and well before this all started. Uro still producing the same number of wet nappies.
