He lives alone in a small apartment and is responsible for his own activities of daily living (adls). Over the past months, mr. chen has been progressively losing weight. He indicates that he has no appetite and eats only one small meal a day. The past week he has had diarrhea as well now struggles to get to the bathroom on time. When mr. chen is admitted to the room his clothing is soiled with feces. The nurse assists mr. chen to remove his clothing, wash, and don a clean hospital gown before getting into bed.
