Person and family centered care (PFCC)
Person centered care moves away form biomedical model and focuses on the whole
person. Focuses the whole person as a unique individual, not just the illness or
disease. Family centered care based on healing relationship and involvement of
family and friends. Includes family, culture and community. Work to understand life
story, experience of health, role of family in person’s life, and role they play to
achieve health. Helps the person achieve goals for their health that they cannot
achieve alone. When they talk about person they are including the family because it
is essential to do that.
Avoid exhausting your volunteers when you ask them about their experience with
PFCC in hospitals/systems.
Family is whatever they decide it is so make sure you understand and value the
diversity and the contributions of all participants
Multiple issues they can affect the PFCC
We are organizing care with the person to develop a plan, and establishing a hared
decision making. You need communication, collaboration and engagement.
