Health assessment - collection of data to develop information base about a patient"s health status (past and present), health concerns, and usual coping mechanisms so that an individualized plan of care can be created (wood & ross-kerr, 2010) One cannot fix a boat based on the request of can you fix my boat? extra knowledge is required such as: Speaking with the one requesting the fix. Observe the boat yourself (interior and exterior) Find someone who can specialize in boats. Prioritize certain fixes over others (e. g. hole in side of boat will be prioritized over the chipping of paint) Health history - patient interview where the nurse collects objective health data. Physical exam - data collected depending on patient acuity, health history findings and current symptoms , with a complete exam including all body systems and regions and a focused physical exam which usually includes 1-2 body systems of concern.
