Everyone thinks they are right: no clear leader with responsibilities, requirements bloat and creep, specifying to many requirements (over stating, will cause problems down the road, collaboration, cost of collaboration. When you decide to work with multiple people on a project, more time is consumed (the cost; not always actual money: the challenge of conceptual integrity. To make it easier to ensure that the stakeholders needs are being met: there is an exception to all of the above; two person teams. Two people can be much more efficient that a bunch of people.
