Figure out what you want to measure, including who your study population is, based on your research question. Try it out on a few people to ensure clarity of questions. People who pre-test questionnaire don"t usually participate in actual data collection. Everyone needs to be able to understand the question; ensure clarity of question. Keep it short, simple, concise - under 20 minutes. Survey questions and instructions that are as closed to interpretation as possible; no issues to interpretation. Think about how you would like to research variables, and build research questions accordingly. Non-response bias: certain type of people who tend not to respond to a certain type of question; certain type of person that is more likely to not respond to a question. Open-ended: no options, just a blank space for people to answer on their own. Factual questions vs. attitude questions vs. knowledge questions. Focus on exactly what you want to know; be specific.
