Did not evaluate the strength and weaknesses. Some members like guides didn"t feel comfortable speaking up or potentially offering a different opinion or challenging an authority. Emotions came in the way(relationships got involved) Contingencies( if something doesn"t work , what"s the plan) Resource problems(not enough oxygen, good radios, etc) Some clients including the leader encountered physical problems. No clear plan to guide the decision being made. It was almost predictable that there are going to be problems. If they could have been more rational and analyzed that the potential risks > potential benefit, they would have stopped.
